Heart Health

Overview:

HeartHealth is a 6-month text message program designed to support patients in improving their heart health. Developed by clinicians at Westmead Cardiology with patient-centred input, the program delivers tailored messages aligned with evidence-based cardiovascular health guidelines. Messages include expert health tips, links to trusted resources, and video content, providing encouragement and practical advice for heart health improvement.

Who is this for:

Patients recently discharged from Westmead Hospital Cardiology or attended Cardiology appointments.

SmilesUp

(Oral Health)

Overview:

Is a 12-week text message initiative providing parents with 36 tailored oral health messages. Developed with consumer and clinician input, it aligns with WHO guidelines and offers reminders, reassurance, and video resources to support families’ oral health behaviour change.

Who is this for:

Parents with children diagnosed with Early childhood caries (ECC) and accessing public dental care in NSW, Australia

PARTICIPATE

(Medical device support)

Overview:

This study aims to examine the efficacy of a novel cardiac implantable electronic device (CIED) remote monitoring patient engagement program into a community setting, focussing on patient CIED management self-efficacy and implementation feasibility.

Who is this for:

People that 18 years or older, owns a smart phone, and have a Biotronik pacemaker or defibrillator that: was implanted within the last month, has remote monitoring capacity, and is equipped with atrial sensing.

TEXTMEPSO

(Psoriasis)

Overview:

Is a 6-month text message initiative providing patients with psoriasis with tailored content aimed at managing psoriasis and keeping a heart healthy lifestyle. Developed with consumer and clinician input, it covers the topics of diet, physical activity, and smoking.

Who is this for:

Westmead hospital patients that have psoriasis.

CHATAF-S

(Atrial Fibrillation)

Overview:

The CHAT-AF-S program is a 3-month digital health intervention designed to help people with atrial fibrillation (AF) manage their condition better. The program uses Artificial Intelligence (AI) to deliver six sessions of automated phone calls, as well as text messages, that provide personalised education and resources to improve knowledge, confidence, and support health behaviours like exercise and medication adherence. This program is being tested in a clinical trial to see if it can improve quality of life and lead to better health outcomes for AF patients.

Who is this for:

Patients who are over 18 years old, have a documented diagnosis of AF, and have a mobile phone that is able to receive calls and text messages.

MICArdiac

(Wearable data)

Overview:

MICArdiac aims to improve customisation of our existing digital health interventions by utilising data collected by wireless monitoring devices (such as step count, blood pressure) and machine learning (artificial intelligence) algorithm to deliver personalised education and remote health monitoring with the goal of improving cardiovascular risk factors. In addition to messages, the MICArdiac program will be delivered as in-app notifications through the smartphone MICArdiac app.

Who is this for:

Anyone who had a diagnosis of hypertension, or high BP, is over the age of 35, has a smart phone and have a good understanding of the English language.
