About the Program

The program is delivered through the DigiCuris platform, where you will receive regular text messages offering tips, educational resources, and reminders to help manage your health. These messages are tailored to your specific condition, lifestyle choices, demographics, and preferences, ensuring that the information is relevant to you.

The program content has been developed by clinicians, researchers and patients building on previous research. Additionally, the content has been developed and reviewed by your hospital doctors. If necessary, the program can also connect you directly to healthcare professionals.

Upon completion of the program, you will be sent a short survey to help us understand what worked for you.

Who can join?

All adults who:

Are aged 18 years and above

Own a smart phone

Have cardiovascular disease

Are currently hospitalised, recently discharged or using cardiovascular care services

CardioCharge is for patients who have been recently hospitalised due to cardiovascular disease. The program supports your recovery by offering personalised advice and resources to manage your heart health, helping you stay on track with treatments and connect with healthcare providers when needed.
